Reorganizes the Rates section of the Agents pricing page so each kind of charge is easier to find.

Per-session charges: conversation, LLM usage, and the phone call surcharge.
Transfers: cold and warm transfer rates in their own table, with a sentence saying they replace the conversation rate and phone surcharge for the rest of the call. The old rows did not say the phone surcharge also stops, and the warm row did not mention the conversation rate at all. The warm row now also says that the hold and briefing before the join bill as part of the conversation.
Monthly fees: phone number rental, moved out of the per-minute table.

The warm transfer note no longer says the joined call is transcribed. The transfers page states that the transcript covers only the agent's segment, up to the merge.

Checked with mint dev: headings and tables render, no LaTeX.
